We are a OHCQ and DDA licensed Agency provider here in the state of Maryland. We provide services in Central Maryland-Anne Arundel, Baltimore, Howard and Harford Counties, and Baltimore City.

Our Mission

Our mission is to provide in-home support, resources and promotion of community integration to adults (21 and above) in need of nursing, home care or diagnosed with intellectual or developmental disabilities, enabling them to participate as fully and independently as they choose in life and in the community. 

 

About Us

We are a OHCQ and DDA licensed Agency provider here in the state of Maryland. We create  individualized PCP and Care of Plan to individuals (aged 21 and above) and provide support and resources for healthy living irrespective of age, sex, ethnicity, or religion. We provide services in Central Maryland-Anne Arundel, Baltimore, Howard, Harford Counties, and Baltimore City.

Patient Bill Of Rights

Our organization is committed to providing quality geriatric care.  It is our pledge to provide this care with respect and dignity.  In keeping with this pledge and commitment, we present the following Patient Rights and Responsibilities:

You have the right to:

  •  Receive accurate information about home care and services provided by our agency.

  • To be fully informed of agency policies and charges for services.

  • To choose a care provider and to receive safe professional care, in accordance with the state of Maryland’s regulations and national standards

  • A personal clinician who will see you on an on-going, regular basis.

  • To be assured of reasonable continuity of care and 24 hours access to care.

  • To be referred to other forms of continuity resources for any treatment or procedures

  • To refuse services within the confines of the law and to be fully informed of the results of your decisions.

  • To receive confidential treatment of personal and medical information.

  •  To be involved in planning for discharge or transfer.

  • Competent, considerate and respectful health care, regardless of race, creed, age, sex or sexual orientation.

  • To have your personal properties treated with respect.

  • To be informed orally and in writing of all services to be provided, the availability, and the extent to which you are financially responsible for prior to receiving care from our agency.

  • To be informed orally and in writing of any change in fees within 30 business days from the date the agency becomes aware of the changes to charges.

  • To have access to all bills and to review your entire medical record upon request.

  • To be informed that the agency maintains a liability insurance coverage.

  • To be informed about the appeal process.

  • To receive instructions regarding what to do in an emergency situation.

  •  To be given in writing, the agency name and phone number. 

  • The information necessary to make an informed decision about any treatment or procedure, except as limited in an emergency situation.

  • Be free from mental, physical and sexual abuse.

  • Humane treatment in the least restrictive manner appropriate for treatment needs.

  • An individualized treatment plan.

  • Refuse to participate as a subject in research.

  • The opportunity to file a complaint should a dispute arise regarding care, treatment or service or to select a different care provider.
